Lichen Planus

Key points

  • Itchy, flat-topped bumps or mouth patches
  • Linked to the immune system
  • Not contagious
  • Mouth patches need regular checks

What it is

Lichen planus is an inflammatory condition of the skin and the moist linings of the body, such as the inside of the mouth. It is thought to be related to the immune system.

It most often affects adults in middle age. Skin lichen planus often settles over months, while mouth lichen planus can last longer and needs regular checks.

Common signs

  • Itchy, flat-topped, shiny bumps — purple, brown or dark on brown skin
  • Common sites: wrists, ankles, lower back
  • White lacy lines or sore patches inside the cheeks
  • Burning in the mouth with spicy or hot food
  • Ridged or thinning nails; patchy hair loss in some people

Things that may be linked

  • Certain medicines (for blood pressure, arthritis or malaria)
  • Hepatitis C infection in some people
  • Dental fillings or materials in some cases
  • Stress
  • Injury to the skin, where new patches can appear

Everyday care that helps

  • Use mild soap and lukewarm water; moisturise the skin
  • Try not to scratch — keep nails short
  • For mouth symptoms, avoid very spicy, salty, acidic or hot foods
  • Keep good mouth hygiene with a soft toothbrush
  • Avoid tobacco and limit alcohol
  • Have regular dental check-ups if your mouth is affected

When to see a doctor promptly

Please get medical help soon if:
  • Mouth sores that do not heal, bleed or change in appearance
  • Painful genital sores
  • Rapid spread of the rash
  • Hair or nail loss
